How does the bubble know which patient is open in Open Dental?

Open Dental tells it, on the same computer only. Select a patient in Open Dental and the bubble shows them: where they are right now, their visit, checkout flags, red flags and family, with I'll take, Chat about visit and Checkout complete.

With your office's Open Dental API keys, the bubble hears from Open Dental directly. Without them, it reads the patient from the Open Dental window's title. Turn following off with Stay on this patient.
